As I joined one of the testservers@vaultmp.com, I didn't have a pipboy. But when I got to megaton, I saw someone afk WITH a pipboy, so I assume its a different gamesave. So I changed my gamesave to default.fos and deleted the previous one. Does that mean everyone will see me with that gamesave, or they just wont see me at all?

They will see you with your customized savegame (and equipment). Until vaultmp supports managing items via script (which will be the case in 1 week or so) using a custom savegame is the only way to sync your inventory right now
But how come I'm able to do missions? Like Ive disarmed the bomb at megaton, does that mean I can also detonate it? Im just really confused lol

You are able to do them (they are not disabled yet), though they aren't synced.

Guys i tried replacing the default.fos but it said that i had a different from the server. I really want to play and have a Pip-Boy and my Equipment. Plus why is the limit 4 Players?
