Are you sure your game is 100% clean, you should not make use of the any exe changer like the 4GB exe. I recommend you to reinstall your game and start fresh.

OK I now have the mod working, thank you. Could you tell me if and how I might go about adding items and enemies to the game or is that possible yet? Also is there a way to save progress to the server and or characters?

Everything you asked for is possible, but not without knowledge of scripting.

Open world open is no open source script and should be written from scratch I don't if it's possible, but if placing NPCS is possible why not items.
